Dental Referral Form
When being referred to Downtown Sleep Dentistry and Oral Surgery it can be for a multitude of reasons which includes requiring sedation or anaesthesia for all dental treatments as well as requiring complex dental procedures that are best done under some form of sedation.
Our patients who visit us at our downtown Hamilton dental office may have, but are not limited to:

Anxiety, fear, or phobia of dentistry and its associated sound and/or equipment

Young children that are unable to cooperate

Difficulty getting numb with local anesthesia

Strong gag reflexes

Movement disorders, inability to be stationary

Physical and Mental disability

Extensive dental disease
